The Limits of Traditional Periodontal Evaluation


A periodontal evaluation typically combines a visual exam, gum pocket measurements, and standard dental X-rays. These tools remain valuable, but each has limitations. Pocket measurements tell us how deep the space is between the tooth and gum, while flat X-rays show bone loss along the front and back surfaces of a tooth but often miss what is happening on the inner and outer surfaces, where some of the most significant bone defects occur.

This is especially true around molars, where multiple roots and complex bone anatomy make two-dimensional imaging difficult to interpret accurately. A traditional X-ray may show a shadow that suggests bone loss without revealing its true depth, shape, or location.

What Cone Beam CT Reveals


A CBCT scan captures the jawbone in three dimensions, allowing our team to view bone levels from every angle around each tooth. This makes it possible to identify vertical bone defects, assess furcation involvement in molars where the roots divide, and measure the exact height and thickness of remaining bone with a level of accuracy that is simply not available from flat imaging.

Because the scan can be sliced and viewed at any point along a tooth's root, we can distinguish between bone loss on the surfaces facing the tongue or cheek and loss on the surfaces between teeth, which often appear differently on a standard X-ray. This distinction matters for treatment planning, since the type, location, and extent of bone loss all influence which periodontal therapies are likely to be most effective.

Identifying Furcation Involvement


Furcation involvement occurs when bone loss around a multi-rooted tooth progresses into the area where the roots separate. This condition is notoriously difficult to diagnose accurately with two-dimensional X-rays because overlapping roots can obscure the defect or make it appear less advanced than it actually is.

Cone beam CT removes that ambiguity. By viewing the furcation area in three dimensions, our team can determine the exact degree of involvement and use that information to decide whether a tooth is a good candidate for regenerative treatment, whether root resection may be appropriate, or whether extraction and replacement offers a more predictable long-term outcome.

Supporting Long-Term Monitoring


Periodontal disease is a chronic condition that requires ongoing management rather than a single treatment event. Cone beam CT can play a role in that long-term monitoring by providing a detailed record of bone levels at a given point in time. If a patient's periodontal condition changes in the future, a follow-up scan can be compared against the original to assess whether bone levels have remained stable, improved, or declined.

This kind of objective comparison supports more informed conversations about maintenance care, additional treatment, or adjustments to a patient's home care routine, since decisions are based on measurable changes in bone structure rather than estimation alone.

Periodontal treatment is not a one-size-fits-all process. Mild bone loss confined to the surfaces between teeth may respond well to non-surgical therapies such as scaling and root planing, while more advanced defects with vertical bone loss or furcation involvement often call for a surgical approach.
